Forms & Policies
Payment Policy:
Fees are due at the time of service. We currently accept Debit, Credit Card, E-Transfer, and Cash.
If a client has extended health benefits, and their carrier accepts electronic submissions, the portion paid by the patient’s coverage will be deducted from the total and the outstanding amount is due from the client at the time of service.
* Should there be a problem with the electronic submission, the patient must pay in full and submit the receipt themselves *
NOTE: Extended health benefits DO NOT pay for a missed appointment. Any fees for late cancellation (less than 24 hours) or for a missed appointment (no show) must be paid in full by the patient prior to their next appointment.
Receipts:
To maintain a high level of professionalism, receipts will only be issued in the name of the person receiving the treatment. They will not be back-dated or post-dated.
Late Arrivals:
If a client arrives late for their scheduled appointment time, the treatment will be shortened and billed at the cost of the originally booked appointment.
Missed Appointments (No Show):
An appointment booked with us is time reserved for you – ONLY for you. We do our very best to always be on time for you when you arrive, and we ask that you do the same for us.
Missed appointments and cancellations have a very real cost to your Therapist.
Therefore, if you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the treatment as booked.
Cancellations:
As previously discussed, your booked appointment time is reserved just for you. A late cancellation or missed appointment leaves a gap in the therapist’s day which could have been filled by another client.
If you need to cancel or reschedule your appointment, it would be fantastic if you could provide 48-24 hours’ notice. With that said, we require a minimum of 24 hours' notice. This is a common courtesy to our therapists and other clients who are on the cancellation list trying to get in for an appointment.
NOTE: We recognize no one is perfect and there are circumstances that are out of your control (sudden illness, family emergencies, etc.), so your therapist may make an exception to the above policies on those rare occasions.
For the health of everyone – myself, yourself, and other patients - Please do not book an appointment if you are not feeling well.
Peaceful Surroundings:
To provide peaceful surroundings during our treatments, we ask clients to mute all cell phones and speak at low levels.
Contraindications:
Some medical conditions require modifications to treatment protocols, while other medical conditions require approval from your doctor prior to receiving treatment. If your therapist/practitioner deems treatment inappropriate for your medical condition and is unable to provide you with treatment, you will not be billed for your appointment.
